This guide describes the roles of the different professionals to make it easier for you and your family to understand who might become involved in your care, what their role is and where to get help and support, should you need it. Advance Care Plan: What matters to me

Our booklet "What matters to me: my wishes for my future care" may also be referred to as an advance care plan. It guides you through a range of personal, medical, financial and domestic questions that can be shared with those important to you.

At Trinity, we openly talk about issues relating to death and dying and this includes encouraging people to plan ahead, and spend time thinking about their plans without feeling rushed. This planner (which may also be referred to as an advance care plan), guides you through a range of personal, medical, financial and domestic questions and can be used to share information with those important to you as your illness advances.
