Your team in the hospice and at home

Our specialist nurses, consultants, and doctors are here to help you cope with any troubling symptoms you may be experiencing as well as the practical and emotional impact of your illness.

Our team of occupational and physiotherapists, counsellors, social workers and many other specialists are here to ensure you receive support and care in whatever way you need it.

Our nurses are your first point of contact and we encourage you to talk to them if you have any questions or concerns.  

Our Community team

For patients living at home, our team is here for you over the phone, by video call and in person, 7 days a week during the day time (including 24 hours a day by phone). Though you may not always see or speak to the same nurse, you can rest assured that whomever you speak with will be aware of your care plan and your preferences.

If you are living with dementia, at any stage of your illness, our Dementia Specialist Nurses are here to provide you and your carers with advice and support about managing your condition and alleviating symptoms such as pain and distress, as well as supporting you with Advance Care Planning. Our Dementia Specialist Nurses also provide training to other health and social care providers to help raise standards of dementia care towards the end of life.   

You may also find it helpful to read our information guide about all of the services providing palliative care at home, of which your Trinity team may be just one. 

The team in the hospice

If you are an inpatient at Trinity, our nurses are available day and night for you to go to with any questions or concerns.

Our nurses and doctors are supported by a variety of other professionals and volunteers. During your time in the hospice, we will support you with washing, getting dressed, and other practical tasks. You will also talk to you about your medications, both on admission and when you leave the inpatient unit. And finally, to make you feel more at home, we will always be happy to make you a cup of tea or coffee, read to you, bring you meals or simply have a chat.
