How Trinity can help you and your family

We're here to give you individualised support to address whatever needs you may have at any given time, regardless of where you receive your care. All of our care is completely free.

What we can offer you

Our specialists provide a range of support and services designed to keep you comfortable, active, independent, mobile and well - emotionally, physically, practically and spiritually.

As well as nurses and doctors, our team includes counsellors, social workers, benefits advisors, occupational therapists, physiotherapists, complementary therapists, dieticians and spiritual care experts.

All of our staff are experienced in supporting people with life-limiting illnesses to manage the challenges that you and your family are facing.

The support you receive might include

  • managing the effects of your illness and treatment difficulties at home and at work,
  • dealing with challenges to living independently and getting around,
  • communicating with those close to you about your decisions for the future. 

Inpatient unit

Our inpatient unit is designed to give you comfort, flexibility and privacy. Around 40% or our inpatients come for a short stay and return home after a relatively short period of inpatient care.

Supporting you when you are living at home

Most of our patients choose to be cared for at home by our Community Services team who provide specialist care to you and your family in the comfort of your own home. You may be visited at home by one of our Community Nurses or Palliative Care Assistants, a member of our Therapies team, a doctor of a member of our Patient and Family Support team.

If you are living at home or in another location within the community, you may also be invited to attend Living Well, our programme of outpatient support. Living Well includes group and 1:1 support and is delivered by our physiotherapists, occupational therapists, counsellors, dietician, social workers, complementary therapists and spiritual care team.

Living Well may serve as an introduction to other Trinity services and palliative care support. We welcome referrals for patients who are frail, elderly, with a life-limiting condition or reaching the end of life. Please note, that this criteria is different to our existing nursing and medical criteria.

Emotional and psychological support                   

It’s natural to experience a range of emotions when you are diagnosed with a life-limiting illness. Coping with the stress and uncertainty can be difficult and sometimes it is easier talking to a trained professional. Our specialist staff provide emotional support and counselling to patients, their families and carers. 

Spiritual support  

Spiritual support can help make sense of the 'big questions', find meaning and provide peace through linking you to what matters most as reflected in your own beliefs and values.

Practical support

Dealing with the practicalities of adjusting to a life-limiting illness can be stressful. Our team can work with you to help you to live independently and stay active and mobile for longer in group or one-to-one sessions and can loan you equipment such as wheelchairs if they are required.

If you are worried about money and how your illness might affect your finances, we can give you information and advice about benefits and financial assistance. You may wish to attend our weekly welfare benefits clinic.

All of the support outlined above is available to all Trinity patients and most is also available to your family, friends and carers, including children.

Compassionate Neighbours

Compassionate Neighbours is a social movement of local people empowered to support one another with the aim of tackling the loneliness and isolation that many people living with a life limiting illness may face.
